To do the best job at improving our program and selecting the best coaches for every team, we really need your inputs as to how the season has been going. We have a new feedback form that we would really like all of you to fill out.

We have tried to simplify and make it concise so that it should only take 5 minutes for you to fill out. We also give you the option to give your feedback anonymously and to give direct feedback to be shared with your coach. We would like to request that everyone take a moment to fill it out.

Please be assured that the feedback you provide is completely confidential (the survey is anonymous although you do have the option to identify yourself in the last field of the questionnaire if you so choose).
