Grade 3-8 Player Evaluation Form

Coaches for grades 3-8 need to fill out player evaluation forms at the end of each season.  Please email the completed form as an attachment to the the club administrator Sue DeNatale and to your age group coordinator by deadline dates communicated to you at the end of each season.  In general, forms will need to be submitted by Nov. 15th & May 10 for the fall and spring seasons respectively.

Please email it to admin@wellesleysoccer.org or fax to 781-239-3385 or mail to WUSC, PO Box 81119, Wellesley Hills 02481 before November 15. You should also email it to your age group coordinator.
