Each team should submit one evaluation. Head coach should complete it and he/she may ask for input from assistant coach.

Grade 3-8 Player Evaluation Form
Coaches for grades 3-8 need to fill out player evaluation forms before the end of each season.  Please email the completed form as an attachment to the the club administrator Sue DeNatale and to your age group coordinator.  You can also fax it to Sue at 781.329.0955 (In general, forms will need to be submitted by Nov. 10th & May 10 for the fall and spring seasons respectively.) Select either Excel or Word document.

Grade 2 Player Evaluation Form
Coaches for grades 2 need to fill out player evaluation form  at the end of each season. with Please email the completed form as an attachment to the the club administrator Sue DeNatale and to your age group coordinator by the last day of the season.   In general, forms will need to be submitted by Nov. 10th & June 5  for the fall and spring seasons respectively.  Select either Excel or Word document.
